Aims
Use the BioPsychoSocial (BPS) screener to identify at-risk residents and establish Inter-Disciplinary Groups (IDGs).
Background
Fragmented care and lack of holistic resident assessments created unmet biopsychosocial needs.
Methods
Introduced BPS screening tools, IDG meetings for complex care, and co-developed care plans.
Results
Facilitated 90% of care goals (36/40) within 2.7 IDG sessions; reduced time for care stabilization.
Conclusion
Successfully integrated care plans into the Bukit Batok community with ongoing stakeholder collaboration.
Lessons Learnt
Stakeholder engagement and an asset-based community development model improved buy-in.
