Aims
To empower seniors to age well and live well with person-centered care in ECON Care Residence.
Background
Traditional nursing homes are often viewed negatively, necessitating a person-centered care approach.
Methods
Developed a four-phase care model: exploration, admission, living well, and dying well, incorporating multidisciplinary care.
Results
Improvements in RAF and MBI scores (20% and 47% respectively) and positive feedback from residents and caregivers.
Conclusion
Personalized care journeys enhance resident satisfaction and family engagement.
Lessons Learnt
Continuous teamwork and empathy are crucial in implementing sustainable person-centered care models.
