Aims
To build a shared governance team model facilitated by daily physical and virtual multidisciplinary connections led by Integrated Principal Nurses (IPNs) to enhance care coordination and improve patient outcomes.
Background
The post-pandemic demands have exacerbated healthcare manpower attrition, financial implications, and concerns for quality and safe delivery. There is a need to engage and retain experienced nurses and create work flexibility and career advancement opportunities.
Methods
The project involved nurse-led daily clinical huddles, multidisciplinary discussions, and weekly virtual rounds for complex cases. IPNs performed regular care plan updates, early family engagements, and nurse-led discharges.
Results
The project commenced on 14th November 2023. Significant increases in anticipatory care referrals were observed, successful nurse-led discharges were achieved, and staff satisfaction scores increased by approximately 20%.
Conclusion
The initiative successfully improved clinical outcomes and staff satisfaction, demonstrating the effectiveness of a shared governance model in healthcare.
Lessons Learnt
Enhanced care coordination and alignment of care goals were achieved. The role-design and empowerment of nurses through nurse-led discharges improved efficiency in resource allocation.
