Aims
To double the percentage of HFrEF patients on target-dose guideline-directed medical therapy (GDMT) within 12 months.
Background
Poor adherence to GDMT for heart failure patients led to suboptimal outcomes and increased healthcare costs.
Methods
Introduced a nurse/pharmacist-led, doctor-supervised multidisciplinary team model with structured workflows and training.
Results
Increased target-dose beta-blockers from 19% to 29% and ACEI/ARB/ARNI from 22% to 33%; clinic capacity increased from 15 to 80 patients/week.
Conclusion
The multidisciplinary model effectively improved outcomes for heart failure patients and is scalable to other chronic conditions.
Lessons Learnt
Empowering non-physician staff improves efficiency without compromising safety and quality.
