Aims
Double the percentage of Heart Failure Reduced Ejection Fraction (HFrEF) patients on target-dose GDMT.
Background
Physician adherence to target-dose guideline-directed medical therapy was poor, leading to suboptimal heart failure care.
Methods
Created protocol-driven nurse/pharmacist-led clinics supervised by a cardiologist; expanded to manage post-MI patients.
Results
Increased capacity from 15 to 80 patients/week; target-dose beta-blockers achieved in 29%; ACE-I/ARB/ARNI in 33%.
Conclusion
Demonstrated scalability and potential applicability to other chronic conditions beyond heart failure.
Lessons Learnt
Multidisciplinary models empower healthcare teams for scalable, effective chronic care management.
