Aims
To overcome care fragmentation, improve outcomes for chronically ill patients, and empower nurses to lead consolidated care delivery.
Background
Fragmented care increases healthcare costs and lowers care quality. Alexandra Hospital piloted a nurse-led approach to streamline care delivery for chronic patients.
Methods
Nurses acted as primary care leads: reviewing patients, coordinating care, leading rounds, and facilitating discharge transitions.
Results
40% reduction in patient length of stay; 65% of nurses reported empowerment; 84% reported increased confidence in leading care rounds.
Conclusion
Successfully redesigned care workflows, enabling nurses to lead multidisciplinary care delivery with measurable improvements.
Lessons Learnt
Empowering nurses to lead care improves patient outcomes, reduces fragmentation, and boosts professionalism.
