Methods
Conducted patient assessments, remote monitoring, home visits, and multidisciplinary care plans; leveraged teleconsultation.
Results
Reduced hospital LOS by 4 days (median); achieved cost savings of 50-70% compared to traditional hospital stays.
Conclusion
NUHS@Home demonstrated feasibility of inpatient substitute care, offering a sustainable model to reduce hospital burden.
Lessons Learnt
Cultural shifts towards home-based care and sustainable funding mechanisms are critical for scaling.
