Aims
Implement a co-management care model for non-surgical orthopaedic conditions.
Background
High demand for orthopaedic surgery visits; many non-surgical cases did not require specialist follow-ups.
Methods
Right-sited non-surgical cases to GPs, providing shared care and patient-centric care plans.
Results
833 patients recruited; 51% attended GP follow-ups; 1% required specialist referrals.
Conclusion
SpineCom Programme has successfully optimized care and won an Outstanding Poster Award (2023).
Lessons Learnt
Empowering GPs and improving patient access through co-management models were key to success.
