Aims
Improve population health outcomes and reduce healthcare utilization through a preventive health management model.
Background
Rapid ageing population in Singapore poses challenges such as frailty, co-morbidities, and increasing healthcare costs.
Methods
Delivered interventions such as health screenings, vaccinations, chronic disease management, and social-environmental programs.
Results
Emergency attendances and hospital bed days reduced significantly (p-value < 0.05); quality of life (EQ-5D) improved significantly.
Conclusion
Preventive health management improved population health and healthcare efficiency through multi-sector collaboration.
Lessons Learnt
Relationship building with participants and community partners was essential for sustained engagement and co-creating solutions.
