Aims
The aim is to expand the MSSW model to encompass a broader range of diagnoses while maintaining the core elements of the AECU, focusing on improving flow within the ES1 and ES2 patient population segments, reducing the length of stay and ED wait time.
Background
The MSSW was introduced to provide acute medical care for selected suitable patients, aiming to discharge within 72 hours through rapid assessment from senior doctors, expedited investigations, and early definitive treatment
Methods
The methodology involves a 'Pull' and 'Push' system to optimize admissions, with A&E maintaining admitting rights while the MSSW team actively identifies suitable patients inED for transfer. The model revolves around dedicated clinical manpower, focused on efficiently managing patient turnaround.
Results
The MSSW demonstrated cost savings and a reduction in ALOS for patients. There was a 20-hour reduction in ALOS and significant cost savings for the hospital.
Conclusion
MSSW serves as a good adjunct to the services offered by the General Medicine department, aiding in improving overall patient flow by reducing the length of stay for suitable patients
Lessons Learnt
Challenges encountered during implementation include ensuring smooth processes and addressing challenges through team chat groups and monthly meetings with key stakeholders.
