Aims
Prevent lower extremity amputations in diabetic foot ulcer (DFU) patients by improving access to multidisciplinary care and rapid interventions.
Background
The high incidence of lower extremity amputations in diabetic patients, particularly in those with DFUs, was leading to negative patient outcomes and high healthcare costs.
Methods
Established a one-stop clinic for DFU patients that included podiatrists, vascular surgeons, and endocrinologists. Improved workflow, quick referrals, and team-based care to optimize outcomes.
Results
Achieved a 40% reduction in major LEA rates and an 80% reduction in minor LEA rates, alongside better control of diabetes and hyperlipidemia.
Conclusion
LEAPP proved effective in reducing LEAs and showed significant cost savings through early intervention and integrated care. The program has now expanded across the NHG network.
Lessons Learnt
Early access to multidisciplinary care and coordinated treatment planning significantly reduced amputation rates and improved clinical outcomes.
