Aims
Increase appropriate glycaemic monitoring in newly diagnosed prediabetes patients via lifestyle counselling.
Background
Lifestyle interventions and 6-monthly glycaemic monitoring recommended to prevent progression to diabetes.
Methods
Conducted intervention using Nursing Care Managers to counsel patients on lifestyle changes.
Results
Glycaemic monitoring within 6 months increased from 55.7% to 64.9% post-intervention (16.5% improvement).
Conclusion
Standardized counselling effectively reduced progression risks, warranting broader implementation.
Lessons Learnt
Enhanced patient awareness and targeted emphasis improved monitoring outcomes, especially in high-risk groups.
