Methods
Conducted sector-wide briefings, developed assessment tools, and implemented standardized interRAI frameworks for community care.

Developed a core set of assessment tools, enabling continuity of care and holistic client evaluation. System successfully onboarded to multiple organizations.
Conclusion
The interRAI system supports seamless and effective eldercare services, enhancing community health management capabilities.
Lessons Learnt
Standardized assessment streamlines processes and reduces duplication. Initial resistance was managed through robust training and support.
