Aims
To evaluate the impact of Activation and Care Empowerment (ACE) consultations on the mental well-being of patients with chronic medical conditions.
Background
There is significant clinical and epidemiological evidence associating chronic diseases with psychological states. However, the emotional and psycho-social components of patients are often overlooked in medical care. Integrated Medical Clinic (IMC) assigns a healthcare team to patients with complex medical needs to address these issues.
Methods
The World Health Organisation Five Well-Being Index (WHO-5) was used to measure the mental well-being of 30 chronic disease patients before and after ACE consultations. Comparisons were made between IMC and non-IMC patients.
Results
93.3% of patients showed improvement in WHO-5 scores after ACE intervention. 67% of IMC patients and 39% of non-IMC patients had at least a 20 percentage point improvement. A higher percentage of IMC patients (83.3%) compared to non-IMC patients (77.8%) had scores above 50 percentage points.
Conclusion
Providing IMC patients with a multi-disciplinary approach in consultations improves their mental resilience and overall mental well-being.
Lessons Learnt
Introducing ACE as part of a holistic approach provides additional value by encouraging positive adjustments to chronic diseases, supporting lifestyle modifications, and improving patient self-awareness and self-efficacy.
