Aims
To improve care and outcomes for elderly patients with hip fractures through an integrated transdisciplinary model
Background
Patients with hip fractures face complex needs requiring medical, surgical, and rehabilitation services. Coordination challenges led to fragmented care and inefficiencies
Methods
Developed an integrated clinical pathway incorporating Enhanced Recovery After Surgery (ERAS) protocols. Focused on medical optimization, timely surgery, and early rehabilitation
Results
Successfully reduced length of stay, lowered medical complications, and improved patient satisfaction. Achieved a low 30-day mortality rate and consistently improved ALOS
Conclusion
Integrated care model with a focus on early surgery, functional recovery, and timely discharge helped to reduce complications and improve overall care efficiency
Lessons Learnt
Team-based care, medical optimization, early mobilization, and patient-centered protocols contributed to improved outcomes and patient satisfaction
