Aims
Achieve zero incidents in falls due to elimination needs and HAPU for Ward B15S by 19 Jul 18.
Background
42% of inpatient falls in 2017 (60 cases) were due to elimination needs; Ward B15S reported 7 falls and 2 HAPU cases.
Methods
Implemented wall clocks and flip charts as reminders for PEER rounds every 3 hours; conducted roll calls and provided instruction sheets.
Results
Zero falls due to elimination needs and HAPU reported by May 2018. Staff satisfaction surveys showed improved awareness.
Conclusion
PEER rounds reduced incidents effectively; systematic training and reminders sustain compliance.
Lessons Learnt
Education, supervision, and visual cues (wall clocks/flip charts) help change staff routines and improve patient safety.
