Methods
New referral criteria targeted high-risk patients; nurse referrals made earlier post-PCI.
Results
Percentage of patients receiving education rose to 73% (17% improvement); declined referrals decreased by 20%.
Conclusion
Timely intervention improved access to care and reduced missed opportunities for education.
Lessons Learnt
Interdisciplinary collaboration and early referrals were key to successful implementation.
