Aims
To increase the proportion of screening of medical patients admitted in ward 71 to help identify those at high risk of venous thromboembolism over the next 6 months.
Background
Internationally, 1 in 4 people die from venous thromboembolism, with 55-60% occurring during or following hospitalization. Asian countries have lower anticoagulation usage and varying VTE rates. Locally, VTE risk is high among acutely ill medical and post-surgical patients.
Methods
Creating awareness among junior doctors, using tools from the MD Cal app, and implementing reminders and a 'Smart Phrase' in the Epic system to ensure regular screening.
Results
Before the intervention, the baseline screening rate was 4.16%. After interventions, screening rates increased to 45% and 68% by the end of January and February 2024, and further to 74% and 89% by the end of March and April 2024.
Conclusion
Every hospitalization is an opportunity for prevention of venous thrombosis. Screening is the first step in identifying patients who will benefit from thrombo-prophylaxis. The project has potential to create hospital data for developing local thrombosis-related guidelines.
Lessons Learnt
Unawareness of policy, tools, and thrombosis risk was a significant barrier. Creating awareness and regular reminders were crucial in improving screening rates.
Additional Information
The project highlights the importance of screening in preventing additional burden of new venous thromboembolism and aims to develop local guidelines based on hospital data.
