Equipped Care Connectors with knowledge, established workflows, and conducted health talks for participants.
Results
Achieved 10% improvement in uptake rates for most screenings and vaccinations, except cervical cancer (missed by 2.3%).
Conclusion
Care Connectors proved effective in increasing uptake rates, with plans to expand the model to more communities.
Lessons Learnt
Collaboration and multilateral communication among stakeholders are essential for synchronizing efforts and improving outcomes.
