Aims
To improve patient and staff experience in the diabetes clinic with a three-pronged approach.
Background
Patients lacked understanding of HbA1c, did not keep personal health records, and faced long wait times in the clinic.
Methods
Introduced height and weight signages, played educational videos in the clinic, and distributed a personalized diabetes health booklet.
Results
Patient engagement improved with a 20% increase in self-measured height and weight records; feedback on educational videos and health booklets was positive.
Conclusion
Improved patient experience and staff workflow efficiency through targeted interventions based on stakeholder feedback.
Lessons Learnt
Human-centered design and management support are critical to understanding and addressing stakeholder needs.
Additional Information
Project won Merit Award at SHM 2019; plan to integrate solutions into wider hospital services.
