To create opportunities for Advanced Care Planning (ACP) conversations among older adults.
Background
Older adults face higher risks of hospitalization and cognitive impairment; ACP allows better care planning.
Methods
Outreach to seniors in rental flats; conducted geriatric assessments; provided ACP education and documentation support.
Results
Conducted 35 ACP sessions; increased engagement with vulnerable seniors.
Conclusion
ACP improves decision-making and care planning for seniors in the community.
Lessons Learnt
Building relationships during ACP sessions is key to fostering trust and understanding.
