Aims
Increase the proportion of major trauma patients requiring Massive Transfusion Protocol (MTP) activation with ROTEM done within 60 minutes from 20% to 100%.
Background
Coagulopathy is a major cause of morbidity and mortality in trauma patients. ROTEM is a viscoelastic assay that provides real-time clot kinetics, improving transfusion decision-making.
Methods
Implemented standardized early ROTEM activation and integrated it into MTP workflows. Conducted PDSA cycles and simulation training.
Results
100% of major trauma patients had ROTEM completed within 60 minutes by Oct 2022, reducing ROTEM turnaround from 84 min to 42 min. Cost savings of ~$58,908 per year due to reduced ICU stays and blood product use.
Conclusion
The implementation significantly improved transfusion efficiency and patient outcomes. Future plans include continued monitoring and protocol optimization.
Lessons Learnt
Standardizing ROTEM protocols and early training ensures sustained improvements in trauma care.
