Aims
Improve the EQ-5D-3L score of residents aged 60-69 by 10% within one year of enrolment.
Background
Care Connector participants had EQ-5D-3L scores 16% lower than the Singapore norm at baseline.
Methods
Engaged residents, administered health/social resource checklists, and connected them to appropriate services.
Results
Average EQ-5D-3L score improved by 13% (from 0.79 to 0.89); exceeded the 10% target.
Conclusion
Connecting residents to trusted Care Connectors and resources improved their quality of life.
Lessons Learnt
Trust-building, teamwork, and collaboration with community partners were critical for success.
