Aims
Reduce documentation time for initial assessments to a median of 15 minutes and achieve 100% usage of the official smart-phrase.
Background
The outdated smart-phrase for initial assessment documentation led to inefficiencies and time-consuming processes.
Methods
Created and implemented a new smart-phrase template for standardized documentation, conducted staff training, and collected feedback.
Results
Reduced documentation time from 82 minutes to 12.5 minutes, with 100% compliance in using the official smart-phrase.
Conclusion
The new smart-phrase template improved documentation time and compliance, with positive feedback from staff.
Lessons Learnt
Streamlining documentation and adopting standardized processes significantly improve productivity and efficiency.
