Methods
Conducted PDSA cycles, reminders, and orientation for medical staff on EMR documentation requirements.
Results
Compliance improved: Problem list updates at admission 100%, discharge >90%, and medication reconciliation >95%.
Conclusion
Structured training and system reminders can sustainably improve patient handover documentation.
Lessons Learnt
Education, reminders, and structured interventions successfully improved EMR documentation compliance.
