Aims

Background
Late discharges caused delays and inefficiencies in the geriatric ward due to communication breakdowns.
Methods
Piloted a daily multidisciplinary board meeting in Ward 67 with visual tools for task tracking.
Results
Early discharges improved from 12.5% to 19.5%; readmission rates dropped from 24.8% to 18.95%.
Conclusion
The board meeting model enhanced discharge efficiency, reduced readmissions, and streamlined workflows.
