Aims
To improve self-efficacy in pain management for chronic pain patients and reduce Emergency Department visits and hospital admissions due to poorly controlled pain.
Background
Chronic pain results in poor mobility, multiple hospital visits, and patient dissatisfaction. Poor pain management leads to social isolation and financial concerns.
Methods
Implemented nurse-led telephonic support for 27 chronic pain patients, focusing on medication adherence, pain control, and addressing concerns through follow-up calls.
Results
Achieved significant improvement in medication adherence and reduced Emergency Department visits while stabilizing pain-related outcomes.
Conclusion
Nurse-led interventions stabilized chronic pain outcomes, reduced Emergency visits, and improved patient satisfaction.
Lessons Learnt
Engaging patients with proactive follow-ups and education increases self-efficacy and satisfaction while reducing unnecessary healthcare utilization.
