Aims
Reduce emergency department visits and hospital admissions for chronic pain patients through a telephonic nurse-led support program.
Background
Patients with chronic pain frequently visit hospitals due to pain crises, limited mobility, and poor treatment adherence. The initiative builds on a 2019 virtual pain clinic project.
Methods
Implemented a six-month nurse-led telephonic support service, tracking pain severity, medication adherence, and hospital visit frequency.
Results
Pain severity improved, with 7.4% reporting a shift from moderate to mild pain. ED visits and hospital admissions significantly reduced post-intervention.
Conclusion
The intervention successfully reduced hospital reliance and improved self-efficacy in pain management. Future expansion plans include community partnerships.
Lessons Learnt
Consistent follow-up, medication education, and pain coaching improve patient confidence and self-management.
