Aims
Improve the patient handover process between inpatient wards and radiology to enhance patient safety and increase accountability.
Background
Current handover processes were inefficient, leading to lapses in communication between inpatient wards and radiology, which could affect patient care.
Methods
Developed a standardized handover process and implemented a digital system to track key patient information such as spinal nursing and hourly monitoring.
Results
Increased the percentage of CT/MRI cases with proper notification to radiology by 50%, improving patient safety and accountability.
Conclusion
The new process improved patient accountability and safety, and the changes will be further spread across other hospital departments.
Lessons Learnt
Effective communication between departments and clear, standardized procedures are critical for improving patient safety during transfers.
