SACH faced challenges in driving value-driven care due to fragmented workflows and incomplete data.
Methods
Implemented NUHS Value-Driven Outcome (VDO) framework, data visualization tools, and PDSA cycles.
Results
CQI improved by 26.1% for Hip Fracture and 23.8% for Stroke patients; shortened LOS and wait times; achieved $553K cost avoidance annually.
Conclusion
VDO framework implementation led to significant clinical improvements, cost savings, and sustainable outcomes.
Lessons Learnt
Combining data sources, addressing incomplete data, and fostering stakeholder collaboration drive improvements.
