Aims

To redesign chronic care delivery.

Background

NHGP extended its teamlet care model during the COVID-19 pandemic to implement tele-consultation services across six polyclinics for chronic condition management.

Methods

A multidisciplinary workgroup developed tele-consultation processes through rapid PDSA cycles, automated patient selection, mass actualization, and medication delivery systems.

Results

Conducted 139,845 tele-consultations, reducing 14,000 physical visits per month. Clinical outcomes for tele-consultations were comparable to in-person care, with high patient satisfaction.

Conclusion

Tele-consultations effectively enhanced chronic care delivery, demonstrating comparable clinical outcomes, system savings, and high patient acceptance.

Lessons Learnt

Leveraging IT from the start is crucial. Multi-disciplinary teamwork and communication drive innovative solutions. Patient perceptions of telehealth must be continuously evaluated.
