Aims
Design a structured process for facilitating decision-making for terminally ill patients on their preferred plan of care during their hospital admission.
Background
Previously, decision-making was solely left to the Next of Kin (NOK), often causing distress. Patients frequently underwent aggressive medical interventions despite bleak recovery chances.
Methods
Introduced structured PPOC discussions with patients, NOK, and healthcare teams, documented care preferences in electronic health records, and improved communication.
Results
80% completion rates for Code Status, especially among cardiology patients. NOK had reduced emotional burden, and patients received care aligned with their values.
Conclusion
Integrating PPOC discussions into routine care enhances patient autonomy and dignity. Future plans include wider adoption and improved documentation processes.
Lessons Learnt
Early discussions improve end-of-life care decisions, reduce unnecessary interventions, and support caregivers.
