Background
NTUC Health Senior Day Care aligns with Healthier SG, shifting focus to preventive care. Community Nurses empower seniors through health education, coaching, and goal setting to improve long-term health outcomes.
Methods
Community Nurses developed a health education menu, protocol, and goal-setting chart across 26 Senior Day Care centers. Goal setting and structured interventions were used for seniors managing chronic diseases.
Results
38 seniors with diabetes mellitus and 31 with hypertension participated. Self-care techniques improved, reducing acute emergencies and increasing staff satisfaction due to fewer complications.
Conclusion
Sustainability is ensured through continuous audits, community nurse training, and quarterly reviews to tailor self-care strategies.
Lessons Learnt
Ongoing training and feedback loops enhance self-care strategies. The therapeutic nurse-client relationship fosters long-term engagement in self-care.
