Aims

Background
Unplanned ICU readmissions were caused by inadequate assessments, delayed escalations, and suboptimal management in general wards.
Methods
Implemented ICU outreach services for post-ICU patients, invasive line tracking, tracheostomy care, and Code Red triggers.
Results
Reduced ICU readmissions; increased early identification and intervention for high-risk patients.
Conclusion
ICU outreach services improve outcomes, reduce complications, and enhance the care continuum for post-ICU patients.
Lessons Learnt
Regular training and clear workflows ensure early interventions and seamless patient transitions.
