Aims
The aim of the Bukit Batok Township Model of Care (BBTMoC) is to shift towards proactive preventive care, increase care integration and coordination, and partner with the community.
Background
The background highlights the demographic aging in Singapore and the associated increase in healthcare burden due to frailty and chronic morbidity. The BBTMoC was piloted to address these challenges.
Methods
The methodology involved enrolling participants aged 40 and above in Bukit Batok SMC into the BBTMoC. The model is risk-stratified, integrating medical and social care for high-risk participants and enrolling all participants in a preventive health plan.
Results
The results showed decreased hospital utilization, increased health-related quality of life, and higher participation in health screening and vaccinations among participants.
Conclusion
The success of BBTMoC is evident in its adoption at the cluster level and its impact on reducing hospital utilization and improving quality of life.
Lessons Learnt
A vital lesson was the value of co-creating solutions with stakeholders to build trust and overcome care fragmentation.
