Methods
Standardized triage pathways, virtual stroke reviews, and transfer templates.
Results
Reduced acute LOS from 10.47 to 8.17 days; JCH acceptance rate improved from 47% to 87%.
Conclusion
Implementation improved functional outcomes; scalable to other public healthcare institutions.
Lessons Learnt
Standardized pathways enhance communication and care quality.
