Aims
To improve early discharge rates at 11:30 AM and 3:30 PM for Ward 63C patients within 5 months.
Background
Current discharge workflows caused delays, impacting bed availability and leading to patient dissatisfaction.
Methods
Introduced the Discharge Nurse (DN) role, created discharge tools, and optimized workflows.
Results
Discharges before 11:30 AM increased from 6% to 15%; saved 546 hours, equivalent to $2,042.40 annually.
Conclusion
Early discharge improves patient flow and satisfaction; scaling initiatives across wards is recommended.
Lessons Learnt
Clear roles and tools for discharge processes reduce delays and enhance operational efficiency.
