Aims
Provide follow-up care for hip fracture patients post-discharge via telephonic follow-up, ensuring ongoing patient monitoring and treatment adherence.
Background
Hip fractures are common in older populations, and the post-discharge follow-up is essential to manage functional recovery and prevent further fractures.
Methods
Implemented telephonic follow-up at 6 and 12 months post-surgery to assess patient condition, functional status, rehabilitation progress, and adherence to osteoporosis treatment.
Results
Achieved improved functional outcomes (e.g., 90% improvement in Activities of Daily Living, MBI score ≥10 points), with better adherence to osteoporosis treatment and fewer re-admissions.
Conclusion
Telephonic follow-up significantly improved patient satisfaction, reduced hospital readmissions, and enhanced patient engagement in managing their condition.
Lessons Learnt
Extending care beyond acute care to community-based care improves patient outcomes and enhances the understanding of long-term risks.
