Duplication of therapy services in acute and community settings increased waiting times.
Methods
Formed workgroup, developed discharge criteria, and implemented discharge workflows with post-discharge surveys.
Results
22.7% of patients successfully discharged; waiting times for physiotherapy reduced.
Conclusion
Successfully reduced fragmented care and physiotherapy waiting times through right siting.
Lessons Learnt
Improved workflows and communication between acute and community care enhanced service delivery.
