Reduce fall rates of cognitively impaired patients by 6 months through cohorting strategies.
Background
Falls among cognitively impaired patients in Ward C11 (5 falls in 2 months) caused staff stress and risks to patient outcomes.
Methods
Implemented cohorting workflow, staff training on cognitive impairment assessment, and regular audits.
Results
Fall rates reduced significantly across all wards by June 2021; nil falls reported in several months post-implementation.
Conclusion
Cohorting cognitively impaired patients improved patient safety and reduced staff workload.
Lessons Learnt
Staff awareness, cohorting protocols, and regular audits significantly reduced fall risks.
