Aims
To evaluate the use of Flash Glucose Monitoring (FGM) in improving glycemic control for Type 2 diabetes patients transitioning from hospital to home.
Background
SMBG compliance is poor due to inconvenience, potentially leading to suboptimal diabetes management.
Methods
Conducted randomized controlled trial with two groups (FGM vs. SMBG) and analyzed HbA1c and patient satisfaction.
Results
HbA1c improved significantly with FGM (4.9%), showing non-inferiority to SMBG; patients preferred FGM for convenience and utility.
Conclusion
FGM is a viable alternative to SMBG for managing glycemic control in Type 2 diabetes during transitional care.
Lessons Learnt
Patient satisfaction and real-time feedback enhance adherence and glycemic outcomes in transitional care.
