Reduce inpatient fall rate in Rehab Wards from 1.8 to 1.0 per 1,000 patient days.
Background
Fall incidents in rehab wards were higher than acceptable rates. Implementing targeted interventions was essential.
Methods
Used root cause analysis, Pareto charts, and PDSA cycles to identify issues and implement improvements.
Results
Fall rate reduced to 0.99 per 1,000 patient days. Cost avoidance of $23,394 annually due to reduced falls.
Conclusion
Implemented a sustainable workflow to ensure ongoing reduction of fall incidents in rehab wards.
Lessons Learnt
Every healthcare worker plays a crucial role in preventing falls. Continuous training and patient education are key.
