Investigate factors associated with patient enablement in chronic care patients in Singapore.

Patient enablement measures the ability of patients to understand and manage their conditions; critical for chronic care outcomes.
Methods
Mixed methods: Patient Enablement Index (PEI) questionnaire (150 patients) and focus groups/interviews exploring enablers and barriers.
Results
Mean PEI score: 4.5 ± 4.4; higher among patients in specialized clinics; factors include doctor-patient relationships, continuity of care, and patient trust.
Conclusion
Patient enablement scores in Singapore align with Asian contexts but are higher than Western benchmarks.
Lessons Learnt
Holistic care, good advice, and efficient systems are vital to improving enablement; gaps in knowledge impact follow-up enablement.
