Reduce delayed discharges awaiting Allied Health Professionals (AHP) review by 80%.
Background
Delayed discharges in Orthopaedics caused by pending reviews from PT/OT/MSW increased waiting time and reduced bed turnover.
Methods
Communication workflows improved through chat groups, re-education of reporting processes, and regular teamwork feedback loops.
Results
Delayed discharges reduced from 13.2 cases/month to fewer than 2 cases/month; morning discharge compliance improved.
Conclusion
Collaborative communication models significantly reduced delayed discharges.
Lessons Learnt
Communication and teamwork among stakeholders were critical to achieving timely discharges.
