Aims
Reduce ALOS from 12.3 days (Aug 2022) to 9.81 days (Aug 2023) by optimizing discharge processes.
Background
Patients medically cleared for discharge remained hospitalized due to system inefficiencies, increasing bed occupancy rates and healthcare costs.
Methods
Introduced the EXPEDITE Discharge Huddle (nurse-led) and improved multidisciplinary communication, including setting medical clearance dates and early discharge discussions.
Results
ALOS reduced to 9.1 days (3.4-day improvement). 91.8% of patients had documented medical clearance dates, and discharge planning was initiated within 3 days of admission.
Conclusion
The project successfully improved discharge processes. Sustainability is ensured through the appointment of a Discharge Care Navigator (DCN) to lead planning efforts.
Lessons Learnt
Multidisciplinary collaboration and clear discharge planning reduce hospital stay durations and improve efficiency.
