Aims
To standardize the feeding management pathway for Ward B12S patients and ensure 100% accurate nurse knowledge on the pathway.
Background
Inconsistent feeding practices led to risks like overfeeding, underfeeding, and inappropriate supplement use, compromising safety and nutrition.
Methods
Developed a feeding algorithm, conducted training for nurses, and used questionnaires to ensure compliance.
Results
Compliance rate improved to 80% by June 2021; all nurses demonstrated 100% knowledge of the pathway by February 2021.
Conclusion
A standardized feeding management pathway improved patient safety and nutritional outcomes effectively.
Lessons Learnt
Stakeholder buy-in is critical for identifying key issues and ensuring smoother implementation of solutions.
