Aims
The aim of the project is to enhance patient safety and efficiency in the ECG management system by reducing invalid results and improving operational efficiency.
Background
The IntelliSpace ECG Management System (iECG) was implemented across SingHealth institutions in late 2020 to streamline ECG result management. However, issues with incorrect patient information selection led to an average of 35 invalid results per month, affecting patient care and operational efficiency.
Methods
The project involved a series of interventions, including staff education, ECG cart enhancements, and system reconfigurations. A comprehensive root cause analysis was conducted using a cause-and-effect diagram to identify key factors contributing to the issue. The interventions were implemented through Plan-Do-Study-Act (PDSA) cycles.
Results
The interventions resulted in a reduction of invalid results from an average of 35 to 0 cases per month, maintained for over six months. This improvement eliminated time wasted on error reconciliation, enhanced patient care quality, and increased overall operational efficiency.
Conclusion
The project demonstrates that targeted technological enhancements, combined with staff education and process improvements, can significantly reduce errors and improve efficiency in healthcare settings.
Lessons Learnt
Key lessons include the importance of root cause analysis, multi-disciplinary collaboration, and user-centric design. The balance of technical and human solutions proved critical for success.
