Aims
Reduce hospital admissions and length of stay (LOS) for high-touch patients by 10% through improved continuity and coordination of care.
Background
Singapore's ageing population has multiple comorbidities, leading to fragmented care and frequent ED visits, hospital admissions, and poor outcomes.
Methods
Empanelled patients under a generalist physician with a multidisciplinary team, electronic tagging for ED alerts, and dedicated phone support.
Results
Admissions reduced from 1.92 to 0.58; ED visits reduced from 1.96 to 0.77; bed days dropped from 8.3 to 2.08. Estimated cost savings: $471,403.90.
Conclusion
ECCP significantly improved continuity of care, reduced hospital utilisation, and enhanced health outcomes for high-risk patients.
Lessons Learnt
Patient engagement, team-based care, and proactive communication improve care outcomes. Relationship-building fosters trust and coordination.
