Aims
To improve elderly patients’ ability to self-manage chronic pain, extending clinic visit intervals by 100% over 6 months.
Background
Elderly patients with chronic pain frequently visit clinics due to comorbidities, mobility impairments, and poor medication compliance.
Methods
Nurse-led telephonic reviews, patient education, and tailored interventions to enhance pain management and compliance.
Results
Increased clinic visit intervals by 185% (from 5.36 to 15.29 weeks); 0 ED visits for pain; savings of SGD 2,091 for 17 patients over 6 months.
Conclusion
Effective nurse-led telephonic reviews for chronic pain patients improve healthcare system efficiency and patient satisfaction; suitable for wider implementation.
Lessons Learnt
Regular nurse-led follow-ups and patient education significantly enhance self-management, reduce costs, and improve quality of life.
