Background

Heart failure patients often experience recurrent hospitalizations due to poor self-management and lack of community support.

Methods

Implemented a strengths-based empowerment model with structured workshops, goal setting, and regular follow-ups to build patient confidence and self-care abilities.

Results

Improved quality of life scores (EQ-5D), reduced readmissions by 20%, and increased patient satisfaction with care.

Conclusion

The empowerment model proved effective for improving outcomes in community heart failure care.

Lessons Learnt

Patient-centered care and empowerment lead to better clinical and psychosocial outcomes for heart failure patients.
